What Hospice Support Means
Hospice care is a comfort‑focused approach designed for individuals in the final stages of life.
It shifts the goal from curing illness to honoring dignity, easing discomfort, and supporting both the patient and their loved ones.
Hospice support is not about giving up, it’s about ensuring that every moment is as peaceful, meaningful, and comfortable as possible.
For families, hospice support means having a skilled nurse who can guide you through symptoms, medications, and daily care needs with compassion and clarity. For patients, it means receiving gentle, attentive care that respects their wishes, routines, and comfort.
“As a mobile concierge nurse, I bring hospice support directly into the home, creating a calm environment where your loved one can remain surrounded by familiarity and family. My role is to manage symptoms, provide skilled nursing care, and offer emotional support so you never feel alone in this journey.”
-Rosa Duarte BSN, RN, CCRN

What to Expect During Visits
-
I begin by checking in with both the patient and the family, listening closely to concerns, changes, or needs.
My goal is to create a peaceful environment where everyone feels supported.
-
I evaluate symptoms such as pain, breathing, anxiety, restlessness, skin integrity, and overall comfort.
These assessments guide the care I provide and help ensure your loved one remains as comfortable as possible.
-
This may include medication support, wound care, catheter care, repositioning, hygiene assistance, or other comfort‑focused procedures.
Every action is performed slowly, respectfully, and with your loved one’s dignity at the center.
-
I explain what I’m seeing, what to expect, and how you can help keep your loved one comfortable between visits.
Families often find reassurance in understanding what’s happening and knowing they’re doing everything right.
-
Hospice is a tender time. I’m here not only as a nurse, but as a steady, compassionate presence for the entire family.
Whether you need reassurance, a listening ear, or help navigating difficult moments, you are never alone.
-
Before leaving, I make sure you know what to watch for, when to call, and what support is coming next.
Every visit is tailored to your loved one’s changing needs.
Family & Caregiver Support
Hospice is a journey shared by the whole family, and you deserve just as much care and compassion as your loved one.
My role is to support you through every step — emotionally, practically, and with clear guidance you can rely on.
Caring for someone at the end of life can feel overwhelming, but you are not expected to do it alone. During each visit, I take time to listen, answer questions, and help you understand what changes to expect.
I show you simple comfort measures, explain medications, and help you feel confident in the care you provide between visits.
You will always have a calm, steady presence to turn to. Whether you need reassurance, education, or simply someone to sit with you during difficult moments, I am here for you.
My goal is to ease your burden, strengthen your confidence, and ensure you feel supported, informed, and never alone.
Together, we create a peaceful environment where your loved one can be comfortable and where you can feel held, supported, and guided with compassion.
Every Family’s Journey is Unique.
